We present a case of acute PH due to pulmonary embolism for laparoscopic low anterior resection. A thorough preoperative assessment followed by invasive hemodynamic monitoring was done. Factors precipitating pulmonary vasoconstriction were avoided, and intravenous nitroglycerine infusion was used to control PH perioperatively.
