Intravitreal voriconazole for treatment of blastomycotic chorioretinitis in a cat

A 4-year-old neutered male domestic short hair cat was presented for blindness, anisocoria and systemic blastomycosis. Ophthalmic examination revealed bilateral retinal detachment and chorioretinitis. The left eye (OS) had a serous bullous retinal detachment and the right eye (OD) a partial retinal detachment with a subretinal granuloma. OS was enucleated and OD treated with an intravitreal injection of 150 μg voriconazole and 4 mg triamcinolone. Histopathology of OS confirmed blastomycotic chorioretinitis. Suppurating pyogranulomatous inflammation was present within the posterior segment with subretinal effusion and retinal detachment. Characteristic yeasts were present within the choroid, the tapetum, the sclera and the optic nerve. After the antimycotic intravitreal treatment, the retina did reattach in OD and vision returned. Chorioretinal scarring remained and a cataract developed. Routine phacoemulsification was performed. Ten months post-intravitreal antifungal treatment, OD was sighted and comfortable. The patient continued treatment with systemic antifungal medication, topical anti-glaucoma and anti-inflammatory drugs. To the best of our knowledge, this is the first case report of the use of intravitreal antifungal medication for treatment of ocular blastomycosis in a cat.

Un chat européen mâle castré âgé de quatre ans est présenté pour cécité, anisocorie et blastomycose systémique. L’examen oculaire a révélé un décollement rétinien bilatéral et une choriorétinite. L’œil gauche présentait un décollement rétinien séreux bulleux et le droit un décollement rétinien partiel avec un granulome sous-rétinien. L’œil gauche a été énucléé et l’œil droit traité par injection intra-vitréenne de 150 μg de voriconazole et 4 mg de triamcinolone. L’examen histopathologique de l’œil gauche a confirmé la choriorétinite liée à la blastomycose. Une inflammation pyogranulomateuse envahissait le segment postérieur, avec infiltration sous-rétinienne et décollement rétinien associé. Des éléments fongiques typiques étaient présents dans la choroïde, le tapis, la sclère et le nerf optique. Après le traitement antimycosique intra-vitréen, la rétine droite s’est rattachée et la vision a été recouvrée. Des lésions cicatricielles choroïdiennes persistaient et une cataracte s’est développée : elle a été traitée par phacoémulsification. Dix mois après le traitement antimycosique intra-vitréen, l’œil droit était voyant et confortable. Le traitement du chat a été poursuivi à l’aide d’antimycosique et anti-inflammatoire par voie générale, et d’antiglaucomateux topique. À notre connaissance, c’est le premier cas rapporté de traitement d’une blastomycose féline par injection antimycosique intra-vitréenne.
