Patients and methods: This randomized study included a total of 53 patients (44 males, 9 females; mean age: 63.7±8.4 years; range, 39 to 83 years) who underwent coronary artery bypass grafting (CABG) for radial artery (RA) harvesting with electrocautery or LigaSureTM vessel sealing system between January 2019 and September 2019. The patients were randomly assigned into two groups. Group 1 consisted of 27 patients operated with the LigaSureTM small jaw sealer/divider and Group 2 consisted of 26 patients operated with electrocautery. The length of the conduit, the amount of intraoperative blood loss, the number of hemostatic clips before and after the anastomosis, the amount of postoperative blood loss, the number of ecchymoses, hematoma and infection are compared. Results: The amount of blood loss from the beginning of harvesting to the end of the closure of the skin of left arm was significantly lower in Group 1 (p=0.004). The difference in the number of hemostatic clips before and after the anastomosis of RA to coronary artery was statistically significantly lower in Group 1 (p<0.001 and p<0.001, respectively). There was no statistically significant difference in the amount of blood loss postoperatively (p>0.6), number of ecchymoses (p>0.1), and postoperative hematoma (p>0.1) and infection (p>0.03) between the groups. Acıbadem Mehmet Ali Aydınlar Üniversitesi Kalp ve Damar Cerrahisi Anabilim dalyi, 34684 ata ehir, İstanbul, trkiye。摘要目的:在本研究中,我们旨在比较电烧灼与LigaSureTM血管密封系统的效率。患者和方法:本随机研究共纳入53例患者(男性44例,女性9例;平均年龄:63.7±8.4岁;在2019年1月至2019年9月期间,39岁至83岁的患者接受了冠状动脉旁路移植术(CABG),通过电切或LigaSureTM血管密封系统获取桡动脉(RA)。患者被随机分为两组。第一组27例采用LigaSureTM小颌封闭/分隔器,第二组26例采用电灼法。比较吻合前后导管长度、术中出血量、止血夹次数、术后出血量、淤血、血肿、感染次数。结果:1组患者从采收开始至左臂皮肤闭合结束失血量显著低于对照组(p=0.004)。1组RA与冠状动脉吻合前后止血夹数(p0.6)、瘀斑数(p>0.1)、术后血肿(p>0.1)、感染(p>0.03)组间差异均有统计学意义。结论:我们的研究结果表明,LigaSureTM血管密封系统是RA采集的更好选择,特别是对于容易出血的患者。
