This case report describes a standardized optome- tric examination in a 78-year-old female patient. Material and Methods. Visual acuity testing, intraocular pres- sure measurement by non-contact tonometry, pachymetry, chamber angle measurement, optic disc tomography by optical coherence tomography (OCT) measurement, slit lamp examination with 90 dioptry (90D) lens, and visual field measurement by automated standardized perimetry were performed. Results. Best corrected visual acuity was 0.8 (logMar 0.1) in the right eye (OD) and 1.0 (logMar 0.0) in the left eye (OS). In- traocular pressure (IOP) measurement showed a mean value from three measurements per eye of OD 18.6 / OS 18.3 mmHg. The vertical cup/disc ratio (CD ratio) was 0.9 in the right eye and 0.8 in the left eye. The nasal and temporal chamber angle was 41° in the right eye and 41° in the left eye. The visual field measurement showed superior defects on both sides as well as an inferior beginning reduction of sensitivity. The patient was referred to an ophthalmologist with the suspected diag- nosis of “normal tension glaucoma”, which was confirmed by the ophthalmologist and treatment was initiated. Conclusion. This case report describes the importance of using a variety of screening devices/methods in glaucoma screening for early detection. 摘要
目的。本病例报告描述了一位78岁女性患者的标准化视光检查。材料和方法。进行了视力测试、非接触式眼压测量、视厚测量、室角测量、光学相干断层扫描(OCT)测量、90屈光度(90D)晶状体裂隙灯检查和自动标准化视野测量。结果。右眼(OD)最佳矫正视力0.8 (logMar 0.1),左眼(OS)最佳矫正视力1.0 (logMar 0.0)。眼内眼压(IOP)测量显示,每只眼OD平均值为18.6 / OS 18.3 mmHg。右眼垂直杯盘比(CD ratio)为0.9,左眼为0.8。右眼鼻颞腔角41°,左眼鼻颞腔角41°。视野测量显示双侧缺陷明显,敏感度开始降低较差。患者因疑似“正常张力性青光眼”被转介至眼科医生,经眼科医生确诊并开始治疗。结论。本病例报告描述了在青光眼筛查早期发现中使用各种筛查设备/方法的重要性。关键词青光眼,正常张力青光眼,视野测量,光学相干断层扫描,眼压测量,视力
