Thirty templates were developed (heart function, recommendations on nutrition and exercise, treatments and medications, and clinical signs related to the disease and risk factor control), and the CVR obtained was greater than 0.80.</p><p><strong>Conclusion: </strong>All information needs were categorized by patients as important or very important. Abstract
