Methods: In the light of rating model of MMSE and ADL, a screening inventory was developed to assess cognitive abilities, daily activities, and mental and personality changes for elderly, which contained 55 items across 11 areas. Two thousand eight hundred and fifteen community elderly were interviewed with the DSI-E; out of them, two hundred and fifteen were retested after two weeks, one hundred ninety-nine were assessed with MMSE, ADL, MMAS, and CCAS. Results: The stability coefficient, split-half reliability, and Cronbach's α coefficient were 0.96, 0.91, and 0.85 respectively; The DSI-E was significantly correlated with MMSE (r=0.92), ADL (r=0.89), MMAS (r=0.71), and CCAS (r=0.63). Factor analysis got three factors accounting for 60.1% of variance, which were named cognitive ability, daily activity, and psychopathology.
