Palmar Fasciectomy and Fasciotomy for Dupuytren Disease

Abstract

Dupuytren disease is a connective tissue disorder of the palmodigital fascia that most commonly afflicts middle-aged males. Although the etiology is unknown, this fibroproliferative disorder results in the formation of nodules and cords that result in joint contractures that inhibit hand function. Minimally invasive treatment options include needle fasciotomy and collagenase injections to separate offending Dupuytren cords. If the latter are ineffective, open fasciotomy or fasciectomy is indicated. Dupuytren disease can be aggressive and therefore treatment success is predicated on an understanding of the pathophysiology and a thoughtful surgical approach.
