We reported a 15-month-old girl born with cleft of the right and polysindactyly of the left foot. The right foot has one central ray deficiency. We performed simple reconstruction of the cleft of the right foot and corrected the polysyndactyly at the same time. The evaluation of the procedures were based on assessments on cosmetic, functional, and roentgenographic results. At 3 month after surgery it showed no widening of the reconstructed foot, no hypertrophic  scarring, and no overlapping of the toes. The functional assessment includes evaluation on gait disturbance and ulceration of the foot, we did not find gait disturbance nor ulceration found.
