Abstract

The total coagulation volume achieved with interstitial laser coagulation depends on various factors (volume per single laser application, number of applications, relative position of the applications to each other or degree of overlap, respectively). Dependent on the surgeon total coagulation volume can be small (low volume coagulation) or large (high volume coagulation) in relation to the prostate volume, despite the same laser and application system and identical irradiation parameters used.

So far, this was never mentioned in literature. Retrospective analysis of published studies using diode laser systems emitting at 830 nm, however, allows to detect difference between low and high volume coagulation. In patients treated for benign prostatic hyperplasia, symptomatic improvement can be achieved with low as well as high volume coagulation. Improvement in urinary flow parameters, however, are clearly more pronounced in high volume coagulation. The advantages of low volume coagulation are the shorter catheterization period and lower complication rate.

The complex tissue effects and consecutive clinical effects taking place in thermal therapies for BPH are not completely understood. So far, in the comparison of studies a standardization of thermal treatment techniques was presumed. Due to variations between surgeons and application modes of the same procedure, this is not the case. Therefore, any future analysis of clinical data and future studies should take these variables into consideration.
