Abstract
Photodynamic therapy (PDT) in Barrett's oesophagus is based on photosensitization of malignant or precancerous lesions prior to light-induced tissue destruction. The anatomic conditions in the oesophagus by formation of mucosal foldings respiratory cardia! and oesophageal movements makes exact illumination hardly realisable. However, exact light dosimetry is important with great influence on the therapeutic effect. A new balloon-application tube with a central position of the irradiation device was developed. It guarantees a regular distance of the fibre to the expanded oesophageal wall and allows exact local dosimetry. With two balloons on the tube an exact anatomic position is possible allowing irradiation without gastroscopic controlling. The transmission of the tube is > 99 % showing no appreciable temperature raise(< 1 oq at the surface of the tube at 100 mW/cm2. The application of the tube through the nose of the patient allows speaking during PDT. Additionally treatment modalities like modified irradiation modes can performed easier because time limitation does not play such an important role as before. In comparison to other gastroscope guided techniques the procedure of irradiation is much more exactly and comfortable for the patients.
