We present a case report of successful surgical treatment of a 6<!--> <!-->cm right iliac transplant patch pseudoaneurysm, diagnosed following an episode of acute obstructive renal insufficiency caused by AP ureteral compression. Treatment consisted, by a transperitoneal approach, in partial AP resection, antegrade termino-terminal renal artery re-implantation in the external iliac artery and right lower limb revascularization with extra-anatomical femoro-femoral crossover 8<!--> <!-->mm PTFE bypass.
