The purpose of this study is to describe the HOTS questions in the Final Year Assessment (PAT) physics at SMA Surabaya, this research is a descriptive study by analyzing the PAT question documents qualitatively and categorizing based on the cognitive levels of analyzing (C4), evaluating (C5), and creating (C6). The research target is the PAT question document for grades 10, 11, and 12 for the 2018/2019 academic year, 2019/2020 academic year, 2020/2021 academic year at SMAN 11 Surabaya. The results of this study indicate that the PAT physics questions at SMAN 11 Surabaya based on the cognitive level classification of the C4 question category dominate the percentage value of PAT questions in the last three academic years, namely 2018/2019, 2019/2021, 2020/2021.
