Efficacy analysis of microsurgical management of the anterior choroidal artery aneurysm: A report of 94 cases

Among them, 90 patients underwent aneurysm clipping with the Yasargil pterional approach, and 4 patients underwent modified pterional approach for decompressive craniectomy. After surgery, all patients were followed up clinically to assess the outcomes based on the Glasgow outcome scale(GOS) system. \n \n \nResults \nA total of 120 aneurysms were successfully clipped in 94 patients. Three patients developed cerebral hemorrhage after operation, including 1 case of epidural hematoma on the contralateral side of the lesion, 1 case of ipsilateral frontal lobe hematoma, and 1 case of contralateral basal ganglia hemorrhage. Seven patients developed cerebral infarction, including 1 case of large-scale cerebral infarction and 6 cases of basal ganglia infarction with paralysis. One case suffered from pulmonary embolism 3 days after operation. Three cases developed hydrocephalus and returned to independent life after ventriculoperitoneal shunting. The follow-up time in 94 patients was 6 to 120 months. Abstract

Objective To explore the efficacy and safety of microsurgical treatment of choroidal artery aneurysm (AChAA). 显微外科治疗脉络膜前动脉瘤94例疗效分析
目的探讨显微外科治疗脉络膜动脉瘤(AChAA)的有效性和安全性。方法回顾性分析青岛大学附属医院神经外科1997年12月至2018年12月收治的94例AChAA患者的临床资料。其中90例采用Yasargil翼点入路夹闭动脉瘤,4例采用改良翼点入路行减压开颅术。术后对所有患者进行临床随访,根据格拉斯哥预后评分(GOS)系统评估预后。结果94例患者共成功夹闭120个动脉瘤。3例患者术后出现脑出血,其中病灶对侧硬膜外血肿1例,同侧额叶血肿1例,对侧基底节区出血1例。7例发生脑梗死,其中大面积脑梗死1例,基底节区脑梗死伴瘫痪6例。1例术后3天发生肺栓塞。3例脑室腹腔分流术后出现脑积水,恢复独立生活。94例患者随访6 ~ 120个月。94例患者中GOSⅣ84例(89.4%),Ⅲ级3例(3.2%),Ⅱ级2例(2.1%),Ⅰ级5例(5.3%)。结论开放显微手术夹闭术仍是AChA动脉瘤的重要治疗方法,充分了解AChA的显微解剖,结合熟练的显微手术夹闭技术,可获得满意的临床效果。关键词:颅内动脉瘤;前脉络膜动脉;外科手术;治疗结果
