Abstract

Objective To evaluate the effect of different hemoglobin (Hb) concentrations on the lung injury in a rabbit model of hemorrhagic shock and resuscitation (HS-R). 目的探讨不同血红蛋白(Hb)浓度对失血性休克复苏(HS-R)兔肺损伤的影响。方法50只2月龄、体重1.9 ~ 2.4 kg的健康雄性新西兰兔,采用随机数字表法分为5组(每组10只):对照组(C组)、HS-R组(H组)、HS-R加灌注浓缩红细胞组R1组(60 g/L≤Hb<80 g/L)、R2组(80 g/L≤Hb<100 g/L)、R3组(100 g/L≤Hb<120 g/L)。复苏后3 h处死大鼠,取肺组织光镜下观察病理变化,再次取肺组织测定干湿比(W/D ratio)、中性粒细胞髓过氧化物酶(MPO)、NO水平及细胞凋亡(TUNEL)。结果与C组比较,其余各组细胞内MAP、NO水平、W/D比值、凋亡指数均显著升高(P < 0.05)。结论HS-R术后维持Hb 80 ~ 100 g/L可减轻家兔急性肺损伤。关键词:休克;失血性;心肺复苏;血红蛋白;急性肺损伤
