Among the 46 cases of events, we analyzed the classifications, causes and links of error and compared those events with adverse events of nursing medication before implementing the information drug close-loop management mode in 2017. \n \n \nResults \nAfter carrying out the drug close-loop management, the adverse events of medication declined compared with that in 2017, but there was no statistical difference (P>0.05) . The adverse events included 17 (17/46, 36.96%) errors in identification, 8 (8/46, 17.39%) medication omissions, 7 (7/46, 15.22%) errors in dose and 5 (5/46, 10.87%) errors in route. The links of medication errors were mainly in drug dispensing (26 cases) and arranging (14 cases) . \n \n \nConclusions \nAdverse events of medication most often occur in drug dispensing. The most common errors were misidentification. Abstract

信息药品闭环管理模式对护理用药不良事件的影响
目的探讨信息药物闭环管理模式对护理用药不良事件的影响,分析实施信息药物闭环模式后发生的46例不良事件,了解事件特点,制定对策,减少护理用药差错。方法回顾性分析厦门大学第一附属医院实施信息药品闭环管理模式后(2018年1-12月)系统上报的46例护理用药不良事件。在46例事件中,我们分析了错误的分类、原因和环节,并将这些事件与2017年实施信息药物闭环管理模式之前的护理药物不良事件进行了比较。结果实施药物闭环管理后,药物不良事件较2017年有所下降,但无统计学差异(P>0.05),不良事件包括识别错误17例(17/46,36.96%),药物遗漏8例(8/46,17.39%),剂量错误7例(7/46,15.22%),途径错误5例(5/46,10.87%)。用药差错的环节主要在配药环节(26例)和配药环节(14例)。结论药品不良事件多发生在配药环节。最常见的错误是误认。护理管理人员应根据护理用药中不良事件的分类和特点制定预防性干预措施,监测和提高药品闭环扫码率,对护士进行力量训练,检查配药和排药情况。关键词:护士;信息化;闭环管理;用药错误;不良事件
