Revisiting the triple aim--are we any closer to integrated health care?

Abstract

Are we, as a nation, any closer to integrated health care? Have the preconditions for the pursuit of the Triple Aim been achieved? Can the U.S. health care delivery system pass the "Triple aim test?".
