Abstract

A unique intervention delivers behavioral health support to patients with less prevalent, but high-cost conditions. Not nearly as many patients suffer from Crohn's disease or rheumatoid arthritis as diabetes, but Newark, NJ-based Horizon Blue Cross and Blue Shield believes it can nonetheless improve care for these types of patients-and recoup an ROI--by providing them with an intervention that emphasizes psychosocial support.
