Abstract

Objective

To examine associations of physical activity (PA) and intrinsic capacity (IC) with incident frailty.

Methods

Adults aged ≥ 50 years from CHARLS (n = 1519) and ELSA (n = 4897) were analysed. PA was questionnaire-assessed; six-domain IC was classified as robust, preimpaired or impaired; incident frailty was defined as Frailty Index ≥ 0.25. Cox models estimated adjusted hazard ratios (HRs).

Results

Robust versus impaired IC was associated with lower frailty hazard in CHARLS (HR 0.42, 95% CI 0.32–0.55) and ELSA (HR 0.40, 95% CI 0.31–0.51). PA was associated with lower hazard in ELSA (HR 0.64, 95% CI 0.55–0.75) but not CHARLS (HR 0.94, 95% CI 0.77–1.16); the PA × cohort interaction was significant (p < 0.001). Active + robust showed the lowest hazard in CHARLS (HR 0.38, 95% CI 0.27–0.55) and ELSA (HR 0.36, 95% CI 0.25–0.53).

Conclusion

Preserved IC was consistently associated with lower frailty risk, supporting joint assessment while accounting for cohort-specific PA measurement.
