Abstract
Many caregivers provide home care for older adults with cognitive impairment, potentially putting their own health at risk. To assess caregivers' self-reported healthcare needs and identify risk factors for caregiver stress. This cross-sectional study included 133 caregiver-older adult dyads involving older adults with cognitive impairment followed at the Centre Gérontologique Départemental 13 in Marseille, France, in 2023. Caregiver needs were assessed using the interRAI SCaN (Self-Reported Carer Needs) comprehensive assessment tool. High caregiver stress was defined as a desire to reduce or discontinue caregiving. Most caregivers were women, with a mean age of 65 years, and 41.4% had been providing care for more than 5 years. Anxiety was reported by 65%, depression by 59%, sleep disturbances by 72%, and daily pain by 57%. High caregiver stress affected 24% of the sample. In multivariable analysis, high caregiver stress was significantly associated with symptoms of depression (OR = 1.6), difficulties providing care in a timely manner (OR = 2.7), and lack of time for leisure activities (OR = 5.5). Caregivers reported substantial mental health difficulties. Factors associated with caregiver stress were multidimensional, while engagement in social and leisure activities may play a protective role. Early comprehensive assessment of caregivers' healthcare needs and support for maintaining leisure activities may help prevent caregiver burnout.
