Abstract

Cutaneous vascular anomalies comprise a heterogeneous spectrum of congenital or early-onset vascular lesions, including capillary, venous, lymphatic, and arteriovenous malformations, as well as selected vascular tumors such as infantile hemangiomas. Aberrant vascular development frequently produces conspicuous lesions with attendant psychosocial, cosmetic, and sometimes functional burden. Over the past several decades, laser- and light-based interventions have become fundamental to management, with progressive refinements yielding notable gains in both efficacy and safety. This narrative review evaluates the clinical performance, safety profile, and recent technological advances of laser therapies for cutaneous vascular anomalies, with particular attention to pulsed dye laser (PDL), neodymium-doped yttrium aluminum garnet (Nd:YAG) lasers, intense pulsed light (IPL), and selected emerging platforms including potassium titanyl phosphate (KTP) lasers and multimodal strategies. Relevant literature was reviewed with emphasis on efficacy by lesion subtype, patient selection, peri-procedural planning, post-treatment care, and complication mitigation. PDL remains the reference modality for many superficial vascular lesions, including port-wine stains and superficial infantile hemangiomas, whereas Nd:YAG lasers offer superior penetration for deeper or bulkier lesions at the cost of greater discomfort and a narrower safety margin. IPL and KTP devices provide additional versatility, particularly for superficial telangiectatic and erythematous lesions. Emerging strategies-including dual-wavelength regimens, next-generation large-spot devices, and adjunctive approaches for treatment-resistant lesions-continue to expand the therapeutic armamentarium. When tailored to lesion morphology, skin phototype, patient age, and anatomic site, laser therapy remains a safe and effective cornerstone in the management of cutaneous vascular anomalies. Ongoing innovation and more personalized treatment algorithms are expected to further improve durability of clearance, reduce recurrence, and refine long-term outcomes.
