Abstract
Background: A 74-year-old female with a history of polycythemia vera on ruxolitinib and type two diabetes mellitus presented with a left calcaneal Coccidioides immitis and methicillin sensitive Staphylococcus aureus Brodie's abscess in the setting of pulmonary coccidioidomycosis. Patient underwent surgical debridement of the abscess with amphotericin mixed cementing of the calcaneal defect and long-term oral posaconazole leading to abscess and sinus tract resolution.
Conclusion: Calcaneal Brodie's abscess is a rare osseous manifestation of disseminated Coc-cidiomycosis requiring surgical debridement and coordinated care with infectious disease. Early recognition, debridement, filling of the abscess void, and prolonged antifungals are paramount in treatment.
