Conservative strategies like casting and orthoses are first-line but limited by high recurrence and compliance challenges. Surgical off-loading procedures-Achilles tendon lengthening, gastrocnemius recession, and lesser tendon recessions-target the root biomechanical cause and demonstrate superior durability in ulcer prevention. Evidence, including research by Laborde and others, supports their role in healing neuropathic and Charcot-related ulcers.
