Abstract
Objective
To translate, culturally adapt, and validate the Spanish version of the Adherence to Exercise for Musculoskeletal Pain Tool (ATEMPT) for use in Spanish-speaking individuals with musculoskeletal pain.
Methods
A cross-sectional psychometric validation study was conducted in accordance with COSMIN and international guidelines for cross-cultural adaptation. The translation process followed a forward–backward method and included expert review and pilot testing. Psychometric analyses included internal consistency (Cronbach's alpha), test–retest reliability (intraclass correlation coefficient, ICC), measurement error (standard error of measurement [SEM] and minimal detectable change [MDC]), and structural validity (exploratory and confirmatory factor analyses).
Results
A total of 201 patients receiving physiotherapy for musculoskeletal pain completed the Spanish ATEMPT questionnaire. For test–retest reliability, 190 participants completed the questionnaire again after 14 days. The Spanish ATEMPT demonstrated excellent internal consistency (Cronbach's α = 0.898) and good test–retest reliability (ICC = 0.770). Structural validity supported a unidimensional model, confirmed by confirmatory factor analysis with excellent fit indices (CFI = 0.996, TLI = 0.994, RMSEA = 0.070, SRMR = 0.036). Measurement error was low (SEM = 1.76; MDC95 = 4.86). No floor or ceiling effects were identified.
Conclusions
The Spanish version of the ATEMPT is a valid, reliable, and culturally adapted instrument for assessing adherence to therapeutic exercise in individuals with musculoskeletal pain. Its implementation may enhance clinical decision-making, treatment planning, and monitoring of rehabilitation outcomes in Spanish-speaking populations.
