We present an unusual case in which the patient developed PTS associated with streptococcal pharyngitis as a child, and his shoulder symptoms did not recover despite ongoing physical therapy. By young adulthood, he developed high-grade glenohumeral joint subluxation on the verge of full dislocation. Furthermore, imaging demonstrated the possibility of a reverse Hill-Sachs deformity.
