Extracts 1 and 2 were split and published as Extracts 1–4. Because of this, the original Extracts 3 and 4 were inadvertently left out.

Additionally, in the ‘Conforming answer followed by non-conforming extension: Accepted by clinicians without pursuit’ section and in Extract 4, ‘XG’ should have been ‘XM’, and their pronouns should have been he/his.

These have been corrected in the article.
