The proportion of western medicine cost decreased by 5.59%, but remained the main composition (31.78%). The health insurance payments, out-of-pocket (OOP) cost per visit, and OOP cost proportion decreased significantly (<i>p</i> < 0.05). The health insurance payments per visit showed a monthly downward trend (<i>p</i> < 0.001), and the decline in OOP cost per visit extended (<i>p</i> = 0.003).</p><p><strong>Conclusions: </strong>The reform reduced the economic burden of COPD inpatients, optimized the cost structure and improved the efficiency of health insurance utilization. Abstract
