The optimal treatment for prolonged survival remains unknown.</p><p><strong>Case presentation: </strong>This study reports a patient with leiomyosarcoma (LMS) diagnosed through postoperative recurrence with failed systemic chemotherapy. The patient underwent retroperitoneal tumor resection and surgical pathology showed LMS.A CT scan of the abdomen was performed one year after the surgical procedure, which revealed the presence of multiple intrahepatic metastases. Two cycles of systemic chemotherapy with a combination of doxorubicin and ifosfamide were performed, after which imaging assessment showed the intrahepatic lesions were significantly enlarged compared to prior ones.
