We describe the case of a 78-year-old man with history of relapsing tonic clonic seizures (TCZ), remote stroke, who presented to emergency department for another episode of TCZ. At blood tests, he had severe hypomagnesemia (0.39 mmol/L). After excluding other causes for hypomagnesaemia, chronic use of PPIs was considered a plausible cause.
