Key outcomes: • Improved MD clinic efficiency using LEAN methodologies to decrease average wait time for patients during clinic visits. • Redesigned the silo structure of Unit Coordinators into a functional team model which has resulted in standardized processes, promoted team work, improved efficiency and morale for staff and provides patients with a more welcoming and supportive environment. • Redesigned the KCC referral form which includes a change in the eGFR criteria to facilitate early intervention in order to slow the progression of CKD. • Revision of the triage criteria to identify and prioritize patients onto urgent, priority and routine pathways, based on eGFR. • Implemented group patient meetings to optimize staff time and to create a supportive patient experience. • Created patient satisfaction survey tools to track progress of improvement. • Created a plan of continuous improvement for future standardization, optimization and support for the KCC patients and staff.
