Abstract

The increasing demand for home care due to the growing number of older people with an increased incidence of multiple chronic conditions requires the adoption of technological innovation that can improve the integration and coordination of health and social services. This paper presents the H@H platform that aims to improve the coordination and cooperation among stakeholders facilitating the provision of care services in a continuity of care framework.
