Abstract
The family health program, created in Brazil by Ministry of Health in 1994 and passed subsequently to the designation of the Family Health Strategy, has as its main purpose to rearrange the practice of health care on new bases and replace the template focused on the disease by a new model centered on the person. This politics prioritizes prevention of diseases and health promotion, in an integrated way and in a defined territory. This monography was focused on the Family Health Program in the city of Glaucilândia / Minas Gerais, proposing a plan of intervention on a high priority problem, the low understanding about Family Health Strategy by health professionals, as well as the population. This plan aims at a better health care. At work is applied the Situational Strategic Planning, with the following critical nodes: (1) low knowledge by the professionals working in the Family Health Strategy, mainly the community health workers, regarding the role of the Family Health Strategy; (2) the absence of strategies to motivate the population to participate in discussions about their health needs and the role of the Family Health Strategy; (3) lack of actions to guide the general public about the role of the family health strategy. For this, three projects are proposed, respectively, (1) training workshops for the family health team, (2) ACS: building knowledge with the community, and (3) Health for all.
