There is an established association between Achilles tendon rupture and corticosteroid or fluoroquinolone use. However, a bilateral rupture in a pregnant patient stands as a rare condition. We present the case of a 28-year-old pregnant female patient with spontaneous bilateral Achilles tendon rupture. The patient had a history of bilateral Achilles tendinopathy. Conservative treatment was initially chosen due to pregnancy. The conservative treatment yielded a good result on the left side. The right side had no clinical improvement and was treated surgically 10 weeks after the injury. The Bosworth technique was used to reconstruct the right Achilles tendon. Clinical outcomes were excellent after two years of follow-up.
