In addition to typical data gathering from paper forms, a dCollective form may include geolocation along with picture, audio and video. Data from each form is extracted to construct longitudinal information pertaining to each patient. Abstract

Collecting effective healthcare survey data requires a novel approach differed from traditional paper-based methods where manual electronic data entry would be required.
