We report a case of extremely rare and atypical lesser sac intestinal herniation through a small defect in the gastro-hepatic omentum. This report details the preoperative and intraoperative diagnostic challenges and shortcomings, and also describe a novel repair technique using fibrin glue to address the culprit defect and prevent future recurrence.
