Khảo sát sự biến đổi tế bào nội mô và bề dày trung tâm giác mạc sau phẫu thuật Phaco trên bệnh nhân đái tháo đường

Abstract

- Đặt vấn đề: Đánh giá xem đái tháo đường ảnh hưởng gì đến lớp nội mô và bề dày giác mạc sau phẫu thuật Phaco. - Mục tiêu: So sánh tế bào nội mô, bề dày giác mạc sau phẫu thuật Phaco giữa nhóm mắc đái tháo đường và nhóm không đái tháo đường. Phân tích tương quan giữa mất tế bào nội mô giác mạc sau phẫu thuật với HbA1c, thời gian mắc đái tháo đường. - Đối tượng và phương pháp nghiên cứu: Nghiên cứu tiến cứu cắt dọc trên 41 mắt của nhóm đái tháo đường và 41 mắt của nhóm chứng có chỉ định phẫu thuật Phaco. Theo dõi trước và sau phẫu thuật. - Kết quả: Tế bào nội mô giác mạc ở nhóm đái tháo đường giảm 6,17% nhiều hơn so với nhóm chứng giảm 4,00% sau phẫu thuật 1 tháng. Bề dày trung tâm giác mạc của nhóm đái tháo đường tăng nhiều hơn nhóm chứng sau phẫu thuật. HbA1c liên quan mạnh với sự giảm mật độ tế bào nội mô sau phẫu thuật. - Kết luận: Đái tháo đường làm tăng sự mất tế bào nội mô giác mạc sau phẫu thuật Phaco. HbA1c có liên quan đến sự giảm này. Abstract - Background: To assess how diabetes affects the corneal endothelial layer and center corneal thickness after Phacoemulsification. - Objectives: To compare corneal endothelial cell and central corneal thickness after Phacoemulsification between diabetic and nondiabetic patients. Correlation analysis between corneal endothelial cell loss after Phacoemulsification with HbA1c, duration of diabetes. - Materials and method: This prospective study included 41 eyes of diabetic patients and 41 eyes of nondiabetic patients who underwent phacoemulsification. To measure before and after surgery. - Result: The corneal endothelial cell losses 1 month after surgery were significantly higher in the diabetic group (6,17%) than in the nondiabetic group (4,00%). The center corneal thickness of the diabetic group increased more than that of the non - diabetic group after surgery. HbA1c showed a significantly correlated with postoperative corneal endothelial density change. - Conclusion: Diabetes increases corneal endothelial cell loss after Phacoemulsification. In the correlation analysis, HbA1c is associated with this reduction.
