La TPN : quelle place dans la stratégie thérapeutique des plaies ?

Abstract

L’intérêt de la thérapie par pression négative (TPN) n’est plus à démontrer, mais ses indications et son positionnement dans l’arbre thérapeutique restent à préciser. À défaut d’un consensus réel sur ce traitement d’utilisation quotidienne, certaines indications peuvent néanmoins être retenues en fonction du type de plaie et de la situation clinique : situations cliniques où elle ne doit pas être utilisée (contre-indications ou non-indications, car sans intérêt ou délétère), situations cliniques où elle a une indication formelle, utilisation transitoire, voire indication exceptionnelle dans certaines impasses thérapeutiques où elle apparaît comme traitement de sauvetage.

The interest of Negative Pressure Wound Therapy (NPWT) does not seem to demonstrate anymore but its indications and its positioning remain to be specified. Failing an real consensus on this treatment of daily use, indications can be established according to the wound and the clinical situation the positioning of the NPWT: clinical situations where it should not be used (contraindications or no indications because of no interest or deleterious), clinical situations where it has a formal indication, transient use or even an exceptional indication in certain therapeutic impasses and it appears as an attempt to rescue.
