摘要

目的探讨糖尿病维持性血液透析(MHD)患者髓过氧化物酶(MPO)水平与颈动脉粥样硬化的关系。方法采用酶联免疫吸附法测定糖尿病MHD患者血浆MPO和血清弹性酶(ELT)水平。b超测量颈动脉内膜中膜厚度(CIMT)、颈动脉斑块数量及面积。结果血液透析(HD)后血浆MPO明显高于HD前(414.6 (198.9671.5)μg/L vs 176.4 (69.9243.3) μg/L,Z =-4.51,P < 0.01)。HD前后血清ELT (198.0(146.9270.5) μg/L vs (2230.9 (40.2308.0) μg/L,Z =-1.87,P > 0.05)差异无统计学意义。年龄与颈动脉斑块数量、面积呈正相关(相关系数分别为0.764、0.416、0.446,P < 0.01或P < 0.05), HD前MPO水平与年龄、HD前血清ELT水平、CIMT水平呈正相关(相关系数分别为0.592、0.476、0.810,P < 0.01)。HD后MPO水平与HD后血清ELT水平呈正相关(相关系数为0.364,P < 0.05)。HD后血浆MPO水平与颈动脉CIMT、颈动脉斑块数量和面积无相关性(P > 0.05)。结论(1)HD患者颈动脉粥样硬化与年龄、HD前MPO水平有关。(2)HD后MPO水平升高与颈动脉粥样硬化无相关性。关键词:血液透析;髓过氧化物酶;糖尿病;颈动脉粥样硬化
