La chirurgie reconstructive dans les hypoplasies du pouce type III de Blauth

L'hypoplasie du pouce du type III selon Blauth reste rare et de traitement controversé. Récemment Manske a défendu la reconstruction dans le soustype IIIA comportant une articulation trapézo-métacarpienne, proposant la pollicisation dans le sous-groupe IIIB où l'articulation basale est absente. Nous avons revu 14 reconstructions du type III, avec un recul moyen de cinq ans. Lorsqu'un transfert microchirurgical de la seconde articulation métatarso-phalangienne est pratiqué, les résultats sont peu différents dans les deux sous-groupes avec même une meilleure stabilité articulaire. Les résultats restent cependant globalement inférieurs fonctionnellement et surtout esthétiquement à ceux de la pollicisation.

Thumb hypoplasia type III according to Blauth remains a rare congenital malformation. Recently Manske has promoted reconstruction versus pollicisation in the sub-type IIIA where a first carpometacarpal joint is present. However we felt that pollicisation is the solution for sub-type IIB where the basal joint is absent. We have reviewed 14 cases of thumb hypoplasia type III, four of them being type IIIB. After performing a first step with a free vascularized second metatarso-phalangeal joint transfer, the secondary steps were identical in both sub-groups. After a mean follow up of five years, no great difference was found in the two sub-groups and basal stability was even better in type IIIB. However the results were functionally and cosmetically inferior to the ones observed after pollicisation. When the relatives refuse pollicisation or the patient consults late for functional improvement, reconstruction remains worthwhile.

La hipoplasia del pulgar del Tipo III de Blauth sigue siendo rara y el tratamiento controvertido. Recientemente Manske defendió la reconstrucción en el subtipo IIIA que comporta una articulación trapezometacarpiana, proponiendo la policisación en el subgrupo IIIB en la que la articulación basal está ausente. Hemos revisado 14 reconstrucciones del tipo III, en una perspectiva de 5 años de promedio. Cuando se practica una transferencia microquirúrgica de la segunda articulación metatarso falangiana, los resultados difieren poco en los dos subgrupos con una mejor estabilidad articular incluso. Los resultados son sin embargo inferiores funcionalmente y sobre todo estéticamente a los de la policisación.
