Blood sample was collected from the patient on admission, and was analyzed at the Department of Biochemistry, BSMMU for estimation of serum Thyroid hormones (FT3, FT4, TSH) level. Modified Rankin Scale (mRS) score was done after 1 month of stroke. 摘要

背景与目的:急性缺血性脑卒中患者神经内分泌谱发生显著改变。该研究的目的是确定急性缺血性脑卒中患者血清甲状腺激素水平与功能预后之间的关系。方法:采用横断面分析方法,在达卡BSMMU神经内科进行。目的选择BSMMU神经内科经神经影像学证实症状出现后14天内的急性缺血性脑卒中患者60例。入院时采集患者血样,在BSMMU生物化学系分析血清甲状腺激素(FT3、FT4、TSH)水平。在脑卒中1个月后进行改良Rankin量表(mRS)评分。不良预后定义为mRS>2,良好预后定义为0-2。结果:60例患者FT3水平正常39例(65%),FT3水平低21例(35%)。不良结局组患者血清FT3水平均值(±SD)为2.77(±0.99)pmol/L,良好结局组患者血清FT3水平均值(±SD)为3.67(±0.65)pmol/L,差异有统计学意义(p=0.001)。mRS评分与血清FT3浓度呈负相关系数(r = - 0.380),差异有统计学意义(p=0.001)。在logistic回归分析中,低血清FT3浓度仍然是急性缺血性脑卒中患者预后不良的独立预测因子(p=0.001,优势比= 3.567)。结论:急性缺血性脑卒中患者血清FT3水平降低与功能预后不良显著相关。孟加拉国神经科学杂志2018;Vol. 34 (1): 9-16
