Anti-IgA antibody was detected from the patient's stored serum collected before the transfusion. It was suggested that IgA present in the transfused platelet concentrate may have induced the anaphylactic shock after reacting with the pre-existing anti-IgA antibody. 摘要

我们报告的情况下,患者与系统性红斑狼疮选择性IgA缺乏谁发展过敏性输血反应。一例50岁系统性红斑狼疮女性患者,入院时免疫球蛋白检查未检出IgA,怀疑IgA缺乏,但未做抗IgA抗体检测。由于术后出血时间延长,给予浓缩血小板输注。开始输血约10min后,患者出现心悸、呼吸困难、低血压并休克。从输血前收集的患者血清中检测抗iga抗体。提示输注血小板浓缩物中存在的IgA可能与预先存在的抗IgA抗体发生反应后引起过敏性休克。选择性IgA缺乏患者应注意抗IgA抗体的存在,以避免意外的过敏性输血反应。
