The etiology for the absence of columellar is diverse, and it is one of the most complex nasal subunits to reconstruct due to its narrow horizontal dimension, its tenuous vascularity, and the limited availability of adjacent tissue.</p></div><div><h3>Aim</h3><p>to present the one-step procedure for the reconstruction of the columella with a chondromyocutaneous graft from the retroauricular region.</p></div><div><h3>Case report</h3><p>A 38-year-old female patient presented with a surgical complication referred by another specialist who, after performing primary rhinoplasty, presented an isolated columella defect.
