Rare cause of acute postoperative endophthalmitis: Gemella morbillorum

We report a case of Gemella morbillorum endophthalmitis after uneventful cataract surgery. A 73-year-old woman presented with a 2-day history of decreased vision, pain, redness, and eyelid swelling in the right eye. She had phacoemulsification with intraocular lens implantation to that eye 5 days previously. The corrected distance visual acuity (CDVA) was uncertain light perception (LP) in the right eye and 20/30 in the left eye. A slitlamp examination shows conjunctival hyperemia, corneal lytic lesions in the upper corneal limbus, a semiopaque cornea with corneal edema, and 3+ cells and dense flare in the anterior chamber. The symptoms did not improve and 2 days after surgery, her CDVA decreased to no LP (NLP). One week after, G morbillorum grew in the specimens. To our knowledge, this is the first report of postoperative endophthalmitis caused by this species that did not respond to treatment and resulted in NLP.
