The usual presentation of such patients is hearing loss and difficulties with balancing. However, we report a case of a 20-year-old female presenting with a long-standing 3rd nerve palsy without any hearing disturbances. Although it is a rare presentation, neuroimaging must be performed in all cases of nerve palsies to rule out any CNS manifestations.
