Amussat's sign in a drowning case: A pitfall to keep in mind

Amussat's sign is a transverse laceration of the intimal layer of the carotid arteries. It is known to be strongly suggestive of hanging. The discovery of this sign in a body recovered from water can arise several questions from a legal point of view. In this paper, we report a case of a drowned body with Amussat's sign. A 41 year-old man was found dead face down in a ten-meter-deep well with a water level of four meters. He has medical history of psychiatric illness and he has stopped his medication in the preceding period. Autopsy revealed Amussat's sign at the level of the two carotid arteries. The medico-legal value and the mechanism of this sign will be discussed.

Le signe d’Amussat est une lacération transversale de la tunique intimale des artères carotides. Il est connu pour être fortement évocateur d’une pendaison. La découverte de ce signe après l’autopsie d’un corps repêché de l’eau peut poser plusieurs problèmes d’interprétation avec des répercussions d’ordre juridique. Nous rapportons, à travers cet article, le cas d’un homme âgé de 41 ans dont le corps immergé a été découvert dans un puits faisant 10 m de profondeur avec un niveau d’eau de 4 m. Il serait suivi pour un trouble psychotique avec une mauvaise observance thérapeutique et en arrêt récent de son traitement. L’autopsie a révélé la présence du signe d’Amussat au niveau des deux artères carotides. Les mécanismes physiopathologiques de ce signe ainsi que sa valeur médico-légale seront discutés à travers ce travail.
