L’obésité est une pathologie dont l’incidence est en augmentation et dont le traitement chirurgical le plus diffusé en France est l’anneau gastrique ajustable (AGA). Le but de cette étude était d’analyser une série d’érosions gastriques ayant compliqué la pose d’un AGA.

Patients et méthodes

Sept patients ont été traités pour des érosions gastriques par AGA survenues dans des contextes très différents, dans un délai moyen d’environ quatre ans et ayant nécessité une intervention chirurgicale pour ablation de l’AGA.

Résultat

Une cœlioscopie a dû être convertie en laparotomie, un patient a été opéré d’emblée par laparotomie. La mortalité était nulle et la morbidité de 57 % (n = 4) : deux épanchements pleuraux et deux infections du site opératoire.

Conclusion

Cette complication rare nécessite une surveillance systématique de tout patient porteur d’un anneau gastrique.

Background

The incidence of morbid obesity is increasing in France; adjustable gastric banding has become the most common surgical treatment.

Patients

We report seven cases of patients who presented with gastric erosion as a complication of gastric banding; this occurred at a mean interval of 4 years following the initial bariatric procedure.

Results

In six cases, repair was performed laparoscopically; one case required conversion to an open laparotomy approach. There was no mortality but morbidity occurred in 57% of cases: pleural effusion (two) and wound abscess (two).

Conclusion

Gastric erosion and migration of adjustable gastric rings can occur at a long interval after laparoscopic gastric banding. Long-term follow-up is necessary in all such patients.
