摘要

目的探讨锂离子治疗对肾功能的影响及影响因素。此外,还研究了尿蛋白/肌酐比值在锂离子肾病检测中的应用。方法测定375例使用锂治疗的患者血清锂浓度、血尿素氮(BUN)、肌酐、肾小球滤过率(eGFR)及尿蛋白/肌酐比值。结果服用锂治疗≥8年的患者BUN、肌酐水平、蛋白尿百分比、2期和3期慢性肾脏疾病(CKD)百分比较高;与服用锂治疗8年的患者相比,尿密度和eGFR更低。0.8和0.6 ~ 0.8 mmol/L组尿密度低于0.6 mmol/L组。CKD的预测因子是血清中锂的含量、锂的剂量、锂的累积使用时间、发病年龄和咖啡因的摄入。结论锂用药≥8年,出现了对肾功能的不良影响。当eGFR为90 ml/min/1.73 m2时,用尿蛋白/肌酐比值测定蛋白尿也能检测到。尿蛋白/肌酐比是一种成本效益高且实用的实验室测试,可用于监测锂治疗患者。

The aim of this study was to investigate the effect of lithium treatment on renal function and to determine influencing factors. In addition, the utility of spot urine protein/creatinine ratio in detection of lithium induced nephropathy was also investigated.
