Chirurgie en un temps d’une exophtalmie basedowienne africaine sévère : présentation d’un cas clinique

Introduction

L’association hyperthyroïdie–exophtalmie, pathognomonique de la maladie de Basedow, nécessite classiquement une prise en charge pluridisciplinaire selon une chronologie précise. Dans certaines régions d’Afrique, les éléments nécessaires à ce type de prise en charge optimale ne sont pas réunis. Lorsque l’état du patient le justifie, on peut alors envisager un protocole thérapeutique raccourci avec traitement chirurgical global en un temps.

Observation

Les auteurs rapportent le cas d’une patiente de 42 ans présentant à la fois un goitre thyréotoxique et une exophtalmie sévère, ayant bénéficié dans le même temps opératoire d’une thyroïdectomie et d’une décompression orbitaire, avec un excellent résultat à 18 mois.

Discussion

À travers cet exemple, les auteurs mettent en évidence l’intérêt de cette approche originale non conventionnelle adaptée aux situations de couverture sanitaire précaire. Elle permet en effet un traitement global en un temps de nature à préserver l’avenir de patients peu aptes à bénéficier d’un suivi régulier.

The association hyperthyroidism–exophtalmia is pathognomonic of the Graves disease. Classically, the treatment is based on a pluridisciplinary step-by-step approach with a precise chronology. In some African places, these optimal conditions are not present. When confronted to such particular situations, it can be proposed to treat surgically at the same time the endocrine disease and the ophthalmologic complications.

Observation

The authors report the case of a 42-year-old women suffering of a great thyreotoxic goiter with severe bilateral exophtalmia, treated by thyroidectomy and orbital decompression in the same session with a good result after 18 months.

Discussion

This non-conventional approach can be useful in particular situations when the medical environment is not optimal. It permits the radical treatment of their disease and improves the prognosis of patients with poor capacities for survey.
