While many international medical graduates (IMGs) traditionally remained in the U.S. after training, an increasing number are now returning to their home countries, driven by personal priorities, technological advances, and evolving global opportunities. This perspective highlights the diverse trajectories of ABR-certified IMGs who have built fulfilling careers outside the West-through teleradiology, academic leadership, curriculum development, entrepreneurial ventures, and health system reform. Their stories underscore how the U.S. training can be adapted to meet local needs, while maintaining professional standards and fostering global collaboration.
