Introduction

Spain is the fourth European country with the highest antibiotic consumption at the community level, and children aged 0 to 4 years are one of the age groups with the highest consumption. The aim of our study was to analyze the evolution of antibiotic prescribing in the paediatric catchment population of a primary care area in Spain according to the indicators established in the National Plan against Antibiotic Resistance (PRAN) over 10 years (2014-2023) and to assess the impact of the COVID-19 pandemic by comparing the 2020-2021 period with the preceding and subsequent periods (2014-2019 and 2022-2023).

Material and methods

Retrospective observational study of the prescribing of antibiotics for systemic use (J01 group of the Anatomical Therapeutic Chemical Classification) in paediatric patients (0-14 years) covered by the primary care system of the Albacete health care administration between 2014 and 2023.

Results

The mean antibiotic prescription was 16.90 defined daily doses (DDD) per 1000 inhabitants per day and 12.10 DOT (days of therapy). The corresponding values in the prepandemic period (2014-2019) were 19.32 DDD/1000 inhabitants/day and 13.92 DOT, which decreased to 8.30 DDD/1000 inhabitants/day and 6.05 DOT during the pandemic (2020-2021) and then increased to 18.05 DDD/1000 inhabitants/day and 12.40 DOT in the pospandemic period (2022-2023). There was an improvement in appropriate prescribing after the COVID-19 pandemic, with a reduction in the consumption of amoxicillin-clavulanic acid and greater consumption of penicillin and amoxicillin during prepandemic and pandemic years (P < .05).

Conclusions

Throughout the study period, there was improvement in the consumption indicators established in the PRAN, with an increase in the use of beta-lactamase sensitive penicillins and amoxicillin and a reduction in the use of amoxicillin-clavulanic acid.
