{"title":"眼轴伸长对近视前巩膜厚度的影响。","authors":"Hui Yan, Jie Li, Cheng Wang, Cai-Qiu Mei","doi":"10.4103/IJO.IJO_444_24","DOIUrl":null,"url":null,"abstract":"<p><strong>Objective: </strong>This study aimed to investigate the effect of axial elongation on anterior scleral thickness (AST) in myopia.</p><p><strong>Methods: </strong>The convenience sampling method was used to select 122 patients without ocular or systemic diseases affecting ametropia in Ya'an People's Hospital between March 2020 and January 2022. According to the diopter and axial length (AL), the patients were divided into an emmetropia group (32 cases), a low-to-moderate myopia group (40 cases), and a high myopia group (40 cases). Relevant data were collected and analyzed.</p><p><strong>Results: </strong>The AST of the four diameter lines was highest at the scleral process and measured. The results, in descending order, were as follows: temporal side (716.45 ± 65.55 μm), lower side (692.32 ± 74.76 μm), nasal side (671.54 ± 61.76 μm), and superior side (550.76 ± 51.34 μm). There was a correlation between the AL and the average AST of the four diameter lines. The correlations, from highest to lowest, were as follows: the mean AST of the nasal side (r = -0.455, P < 0.001), the mean AST of the upper side (r = -0.431, P < 0.001), the mean AST of the lower side (r = -0.402, P < 0.001) and the mean AST of the temporal side (r = -0.363, P < 0.001). Age was associated with the average AST of the upper, nasal, and temporal sides. The correlations, from highest to lowest, were as follows: the average AST of the upper side (r = 0.545, P < 0.001), the average AST of the temporal side (r = 0.422, P < 0.001), and the average AST of the nasal side (r = 0.411, P < 0.001). There were significant differences in AST among the three groups at 0, 1, 2, 3, 4, 5, and 6 mm from the scleral bulge (P < 0.05). AST was positively correlated with age and negatively correlated with AL (R2= 0.432, F = 231.73, P < 0.05).</p><p><strong>Conclusion: </strong>The thickness of the sclera changed with the AL and measurement position. AST was negatively correlated with the AL and positively correlated with age. Compared with emmetropia and low-to-moderate myopia, the anterior sclera in cases of high myopia was thinner. 目的:探讨眼轴伸长对近视眼前巩膜厚度的影响。方法:采用方便抽样法,选取2020年3月~ 2022年1月在雅安市人民医院就诊的122例无眼部或全身性疾病影响屈光不正的患者。根据患者的屈光度和眼轴长度(AL)将患者分为远视组(32例)、低中度近视组(40例)和高度近视组(40例)。收集相关数据并进行分析。结果:4条径线的AST在巩膜突处最高。结果由大到小依次为颞侧(716.45±65.55 μm)、下侧(692.32±74.76 μm)、鼻侧(671.54±61.76 μm)、上侧(550.76±51.34 μm)。4条径系的平均AST与AL有一定的相关性。相关性由高到低依次为:鼻侧AST均值(r = -0.455, P < 0.001)、上侧AST均值(r = -0.431, P < 0.001)、下侧AST均值(r = -0.402, P < 0.001)、颞侧AST均值(r = -0.363, P < 0.001)。年龄与上侧、鼻侧和颞侧的平均AST相关。相关性由高到低依次为:上侧平均AST (r = 0.545, P < 0.001)、颞侧平均AST (r = 0.422, P < 0.001)、鼻侧平均AST (r = 0.411, P < 0.001)。在距巩膜隆起0、1、2、3、4、5、6 mm处,三组间AST水平差异有统计学意义(P < 0.05)。AST与年龄呈正相关,与AL呈负相关(R2= 0.432, F = 231.73, P < 0.05)。结论:巩膜厚度随AL和测量位置的变化而变化。AST与AL呈负相关,与年龄呈正相关。高度近视患者的前巩膜较远视和低中度近视患者薄。近视不仅影响后巩膜厚度,而且影响AST。
